| Family Service Agency of San Bernardino is a non-profit United Way Agency serving San Bernardino families since 1947. The Agency's purpose is to promote and strengthen individual and family life through a variety of crisis intervention, counseling, education and community service programs. We are a member of Alliance for Children and Families. |

| WHO WE SERVE |
| Services offered by the Agency are available to over 300,000 people in San Bernardino County. Annually, the Agency provides counseling services to over 8,000 children, adults and families. Many more individuals are helped through the Agency's education, outreach and intervention programs. No individual seeking services is discriminated against on the basis of ethnic background, religion, or any other status or condition. |

| WHAT WILL IT COST? |
| Fees are based on income. When you call, the secretary will refer you to the Clinic Business Manager. The general nature of your needs for counseling and an estimate of your fees will be discussed. The cost of service is supplemented by Arrowhead United Way, City and County funds, Agency fund-raising activities, and in some cases, your health insurance. |

| IF I DECIDE TO SEEK COUNSELING? |
"Will my privacy be protected?"
Absolutely. Interviews are conducted in private; and matter discussed remain confidential by law. |

| HOW LONG DOES COUNSELING TAKE? |
| Counseling sessions are usually scheduled weekly and are approximately one hour in length. The number of sessions will depend on the seriousness of the problem, how many people are involved, how long the problem has persisted, and your motivation for reaching a solution. Most people are able to reach solutions within 8-10 sessions, some are less and some are more. |

| HOW MAY I MAKE AN APPOINTMENT? |
| Family Service Agency has offices in San Bernardino, Fontana and Crestline. To arrange an appointment, call (909) 886-6737 for San Bernardino, (909) 338-4689 for Crestline, and (909) 822-3533 for Fontana. An initial interview will be scheduled promptly. Evening appointments are available for those who cannot attend during the day. |
